Boiler Troubleshooting — Common Field Issues

Reference guide for facility managers and boiler operators. Always have a qualified boiler technician perform safety-critical repairs.

Burner locks out on ignition — won't light
Check gas supply pressure and shutoff valve. Verify pilot is igniting (listen/feel for click on gas valve). Check spark igniter — should hear continuous sparking during trial. Check flame rod position and condition. Flame signal below 1.25 Vdc on KDM display = amplifier or scanner issue. Check RM7800 KDM fault code for specific diagnosis.

Low water cutoff tripping — boiler shuts down
First: verify actual water level in gauge glass. If water level looks normal — float may be stuck or chamber may be fouled with sediment. Blow down the control and observe if float drops. If LWCO doesn't trip during blowdown, that is a safety failure — shut boiler down immediately. Check equalizing lines for blockage. On 150S/157S: nuisance trips at low pressure = upgrade to MD model.

Boiler leaking — water weeping from handhole
Handhole or manhole gasket failure. If the gasket has been in service more than one season, replacement is the right call — do not attempt to re-torque your way out of a failed gasket. Identify the boiler operating pressure and specify the correct Topog-E® series: Series 180 for under 180 psi steam, Series 450 for up to 450 psi. Using the wrong series for your pressure is a safety issue.

Boiler cycling rapidly — short cycling on pressure
Differential on pressuretrol is too narrow — boiler reaches pressure setpoint quickly and shuts down, then restarts within minutes. Widen the differential on the pressuretrol or operating control. Also check for oversized boiler relative to building load (common in older installations), or excessive piping heat loss. On modulating systems, check setpoint programming and modulation range.

Steam boiler priming / surging water line
Water is carrying over into steam piping — caused by high dissolved solids (TDS) in boiler water, excessive boiler firing rate, or improper water treatment. Check boiler water conductivity — should typically be under 3,500 µS/cm for low pressure steam. If TDS is high, increase surface blowdown frequency. Check near-boiler piping — undersized header or Hartford Loop issues can cause surging that looks like priming.
Surging water line can cause nuisance LWCO trips — probe-type PS-800 with DOB/DOM delay may help.
Annual inspection failed — items to address
Most common inspection failures: LWCO not tripping during blowdown test (replace control), handhole/manhole gaskets showing leakage or age (replace with correct Topog-E® series), burner control not cycling correctly (service or replace relay module), safety valve lifting below set pressure or failing to reseat (replace safety valve — never adjust or wire down a safety valve). Address all items before returning boiler to service.

How often should a commercial steam boiler be inspected?
State boiler codes in Tennessee, Arkansas, Mississippi, Alabama, and Louisiana require annual inspection by a licensed boiler inspector for most commercial boilers. Beyond the code-required inspection, best practice includes pre-season startup inspections, daily blowdowns on low water cutoff controls (per McDonnell & Miller's official maintenance guidelines for commercial applications), annual burner combustion analysis and tuning, and annual internal cleaning of the fireside and waterside surfaces. Burner controls should be functionally tested and flame signal verified at each annual inspection.
What causes a commercial boiler to fail annual inspection?
The most common annual inspection failures we see across the Mid-South: (1) Low water cutoff not tripping during blowdown test — the LWCO must shut down the burner when the water level drops during blowdown; if it doesn't, it's a safety failure requiring immediate replacement. (2) Handhole or manhole gaskets leaking or showing age deterioration — replace with the correct Topog-E® series for your operating pressure. (3) Safety valve lifting below set pressure or failing to reseat — never attempt to adjust a safety valve; replace it. (4) Burner control faults or improper flame signal. What is the difference between a burner control lockout and a low water cutoff shutdown?
A burner control lockout (the relay module requires manual reset) is triggered by the flame safeguard system — typically from a failure to establish flame during the trial for ignition, a loss of flame signal during run, or an internal control fault. The Power LED on the RM7800 will blink a fault code, and the KDM will display a specific fault message. A low water cutoff shutdown cuts power to the burner when the water level drops to the cutoff point — on float-type controls this is a safety switch opening, on probe-type controls the conductivity circuit breaks. Both can look similar from the outside (boiler not running) but have completely different root causes and different fixes.
