Pressure transmitter reading erratically or pegged
Erratic pressure transmitter readings in chemical service are most commonly caused by impulse line issues — plugging with process material, freezing (in cold environments), or liquid accumulation in gas service / gas pockets in liquid service. Isolate the transmitter and equalize pressure: does it read zero correctly? If so, the transmitter itself is likely OK — the impulse lines need to be cleared, drained, or heat-traced. If it still reads incorrectly with equalized pressure, check the calibration and verify the electronic module. In corrosive service, verify the process isolating diaphragm hasn't been attacked by the process fluid.

Control valve not responding to signal
Control valve non-response is almost always in the positioner or pneumatic supply, not the valve body itself. Check first: is there adequate instrument air to the valve? Instrument air supply should be clean, dry, and typically 60-80 psig depending on the actuator. Check the I/P transducer or positioner — is the air output responding to the 4-20mA input signal? If yes and the valve isn't moving, check the actuator and packing friction. If the I/P or positioner isn't responding, verify 24VDC loop supply and that the 4-20mA signal is actually reaching the positioner from the DCS.

High-pressure boiler handhole leaking
High-pressure boiler leaks in chemical plant steam systems are more serious than in commercial applications — operating pressures above 250 psi mean much higher energy release if a joint fails catastrophically. Do not attempt to re-torque a leaking handhole joint while the boiler is under pressure. Take the boiler offline, depressurize, and allow to cool. Replace the gasket with the correct Topog-E® series for the operating pressure: Series 450 for up to 450 psi, Series 1000 spiral wound for higher pressures. Never substitute a lower-rated gasket. Verify the handhole plate seating surface is clean and undamaged before installing the new gasket.

SIS initiator (transmitter) showing fault
A faulted SIS initiator is a safety-critical condition requiring immediate attention under your facility's impairment and bypass management procedure. Do not simply bypass an SIS initiator without proper management of change and impairment authorization. Diagnose the transmitter fault: check loop power (SIS loops often use dedicated 24VDC supplies), verify field wiring continuity (SIS wiring is typically a dedicated home-run to the logic solver), and check the transmitter for process condition issues. If the transmitter requires replacement, the replacement must be a SIL-rated unit of the correct type. Consult your facility's SIS documentation for the approved replacement model.

Level transmitter reading incorrectly — vessel overfill risk
Incorrect level indication in a chemical process vessel can create overfill conditions — a PSM significant incident in many facilities. Verify the reading against an independent level indicator (sight glass or independent transmitter if available). For DP-based level, check both high-side and low-side impulse lines for plugging or liquid fill errors. For guided wave radar (GWR) or radar level, verify the probe or antenna is not coated or obstructed by process material. For displacer-type, verify the displacer hasn't become coated or damaged. Cross-check against process material balance if independent verification isn't available.

Intrinsic safety (IS) barrier showing open circuit
An open circuit fault on a Zener IS barrier typically means the barrier's fusing element has opened — this is a safety function, not a failure mode. Zener barriers are sacrificial devices that protect against dangerous energy entering a classified area. Verify the field device and wiring are not shorted (which would have blown the barrier fuse). If the field wiring and device check out, replace the barrier with an identical unit — the input and output parameters must match exactly to maintain IS certification. Never replace a Zener barrier with a non-IS-rated isolator in a Division 1 application. Consider whether galvanically isolated IS barriers offer a better solution for your application.

What are Class I Division 1 and Division 2 hazardous locations?
Under NEC Article 500, Class I locations are those where flammable gases or vapors may be present in quantities sufficient to produce an explosive or ignitable mixture. Division 1 means the hazardous atmosphere is likely to exist under normal operating conditions (such as inside a tank or near a continuous process vent). Division 2 means the hazardous atmosphere is not likely under normal conditions but may exist during abnormal conditions (such as in areas adjacent to classified areas where leaks could occur). Controls equipment in Division 1 areas must be explosion-proof or intrinsically safe. Division 2 allows some non-incendive equipment under certain conditions. Incorrect equipment in classified areas is a code violation and a genuine explosion risk. We can help specify correctly rated equipment for your location classification.
